June, 1996 Renewal notice from DEA sent to old address, not forwarded, mail forwarding notice had expired.
July 31, 1996  DEA Registration expired
Mar. 1, 1997 DEA Registration retired
1985 - 2003 Alabama Annual Dental Registration renewal asked for the DEA number only, not the expiration date.
Sept., 2003 2004 Alabama Annual Dental Registration renewal asks for DEA expiration date for the first time.  I put ‘unsure at this time’ in the space for the expiration date.
Dec., 2003 I rec’d a 2004 AL Annual Dental Registration Certificate and a 2004 Controlled Substance Registration Certificate from AL Bd of Dental Examiners
Sept., 2004 I submitted 2005 AL Annual Dental Registration without an expiration date
Oct., 2004  2005 AL Annual Dental Registration returned from Board for expiration date.
Nov. 3, 2004 I contacted DEA to determine the status of my DEA Registration – found out my DEA Registration had been retired.  Corrected error with DEA.
I notified AL Board of Dental Examiners of the oversight.
Nov. 5, 2004  I rec’d an initial letter from Board attorney (James Ward) regarding failure to renew DEA registration
Nov. 22, 2004 I rec’d a letter from Board attorney with Consent Order attached.
1st of Dec, 2004 Multiple attempts by me to contact the Board Attorney regarding the Consent Order
Dec. 9, 2004 I rec’d a letter from Board attorney refusing to modify any wording on the Consent Order, “has been signed by all others who committed the same act or omission.”
Dec.14, 2004 I sent a letter to Board attorney challenging the gross negligence charge.  No response. I rec’d a new DEA Registration, no disciplinary action from DEA.
Dec.16, 2004 I am served with Notice papers.

Investigated option through my attorney for appeal of final order.  No recourse to existing law.
Warned by multiple dentists of possible retaliation by the Board attorney.
July, 2005 I rec’d a patient complaint from the Board attorney.  1st complaint ever rec’d.
I referred action to my attorney.
April 2006 AL Board of Dental Examiners drops complaint, not in their jurisdiction.

Organized the grassroots dentists concerned with the actions of the AL Board of Dental Examiners. 
Grassroot dentists elect a reform-minded dentist to the AL Board of Dental Examiners.
Jan, 2008 Grassroot dentists formally organized.  I am elected to serve as the President.
Feb 2008-
April 2008
Submit legislation to force change and bring fairness to the AL Dental Practice Act.  The AL Board of Dental Examiners and the Alabama Dental Association submit opposing legislation.
April, 2008 Legislative session ends with no changes.  Both sides were unsuccessful, no changes made to the AL Dental Practice Act.
June, 2008 AL Dental Association House of Delegates approves our recommendations for changes in the AL Dental Practice Act.
Special Committee established to work out the conflicts between the factions.  I’m appointed to the committee
Oct 1, 2008 Grassroot dentists gets 2nd reform-minded member elected to the AL Board of Dental Examiners
Oct 2008  ALDA Legislative Council approves our recommendations handed down from the ALDA House of Delegates.
Nov. 2008 AL Dental Association’s Board of Trustees approves 90% of our recommendations passed by the Legislative Council
Feb-Mar 2009 I actively negotiated with Steve Windom to finalize bill for passage the State Legislature.
AL State Legislature passes sweeping changes to reform the AL Dental Practice Act through the Sunset process – never has such major changes been allowed to pass through the Sunset Process.
May 2009 AL Board of Dental Examiners releases the Board attorney.
Sept 2009  AL Bd of Dental Examiners releases the Bd’s Administrative Secretary
Oct 2009 Grassroot dentists gets 3rd reform-minded member elected to the AL Board of Dental Examiners
Dec 2009 Executive Director of the AL Dental Association resigns
